Reconstruction of abductor shoulder function with modified Batemen method for trapezius muscle transfer

Abstract:

Background

As a part of the stable structure of shoulder joint, the deltoid muscle has the functions of abduction, flexion and extension, of which shoulder abduction is particularly important. For the malignant tumor growing in the deltoid muscle, deltoid muscle compartment resection is considered as the preferred surgical method. In the meanwhile, after the complete deltoid muscle resection, the abduction function of the shoulder joint will be damaged, and it will also induce dislocation or subluxation of the glenohumeral joint. Therefore, reconstruction of the abduction function of the shoulder joint after total deltoid muscle tumor resection is necessary.

Objective

To investigate the clinical application of modified trapezius muscle transfer in the reconstruction of abductor shoulder function.

Methods

According to the biomechanical principle of shoulder abduction and the anatomical characteristics of trapius muscle, 3 patients with deltoid malignant tumor were reconstructed by using modified Batemen method (using allogeneic tendon on the basis of Batemen method) .

Results

The mean follow-up duration was 12 months, and 3 cases were excellent according to the trial standard of upper limb function evaluation of Chinese Medical Association Hand Surgery Society, and the mean disability of arm shoulder and hand (DASH) score was 4.44 points.

Conclusion

Reconstruction of shoulder abductor function after complete resection of deltoid malignant tumor with modified Batemen method of trapezius muscle transfer is worthy of clinical promotion.
