肩关节是人体最灵活的关节之一,肩关节周围骨折预后直接影响患者上肢功能与生活自理能力。骨质疏松是影响骨折愈合的关键因素,也是危害老年人群运动健康和生活质量的常见病、多发病。与骨量正常的骨折患者相比,骨质疏松骨折患者遗留功能障碍的比例显著升高。当前临床关节周围骨折诊疗过程中,骨质疏松症的危害被严重低估,异常的骨代谢情况直接导致了更高的内固定失败率、并发症风险和二次手术风险,同时极大程度延缓了骨折愈合与功能康复。近期,本刊针对肩关节周围骨折刊发系列文章,在肱骨近端骨折、锁骨远端骨折、肩锁关节脱位等方面深入阐述。笔者综合本中心诊疗经验,针对近年来临床关注的肩关节周围骨折骨质疏松诊疗若干问题进行探讨,为临床同道提供些许参考。
