胸锁关节作为肩带的重要组成部分,其稳定性对于肩部功能至关重要。尽管胸锁关节脱位较为罕见,一旦发生,可能会导致显著的功能障碍,影响患者的生活质量。胸锁关节脱位主要分为前脱位和后脱位,其中前脱位较为常见,通常不涉及生命威胁,可以通过保守治疗或闭合复位有效管理;而后脱位较为少见,但由于涉及重要的纵隔结构(如气管、大血管等),具有较高的生命风险,需紧急干预。治疗方案的选择依据脱位类型、损伤程度及患者的临床表现而定。尽管传统上以保守治疗为主,近年来随着手术技术的发展,尤其是在慢性和不稳定病例中的应用,手术治疗的效果逐渐得到认可。对于慢性胸锁关节脱位,手术治疗成为恢复关节稳定性和功能的关键,常见方法包括锁骨内侧切除术、肌腱移植物重建等。本文综述了胸锁关节脱位的最新治疗进展,分析了各类治疗策略的优缺点,重点讨论了前脱位与后脱位的不同治疗方法。通过对现有治疗方法的总结,本文旨在为临床实践提供理论依据,并为未来的研究提供参考。
