肱骨近端骨折是老年人常见的四肢骨折之一,约占全身骨折的5%,发生率仅次于髋部和桡骨远端骨折。3、4部分骨折(Neer分型)发生率虽然仅占所有肱骨近端骨折发生率的15%,但由于此类骨折移位较大、粉碎明显,因此一直是临床治疗的难点。目前多数学者认为年轻患者的3、4部分骨折应首选手术治疗,但老年患者的治疗方案仍然存在争议。本文就老年人肱骨近端3、4部分骨折的治疗现状、反式全肩人工关节置换术治疗老年人3、4部分骨折的疗效、反式全肩人工关节置换术治疗老年人3、4部分骨折的技术要点、并发症及康复等方面进行综述,并对该术式日后的研究方向进行展望。
